The findings were: 1. Review of the medical record for resident #4 showed s/he was admitted to the hospital for an acute change of condition on 2/26/19 and was readmitted to the facility on [DATE]. Further review showed no evidence the facility issued a written notice of the bed-hold policy to the resident or the resident's representative. 2. Review of the medical record for resident #27 showed s/he was admitted to the hospital for an acute change of condition on 3/19/19 and remained hospitalized at the end of the survey on 3/27/19. Further review showed no evidence the facility issued a written notice of the bed-hold policy to the resident or the resident's representative. 3. Interview with the DON on 3/26/19 at 9:49 AM verified the written notice of the bed-hold policy had not been issued to the resident or the resident's representative.
