In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Two medication errors were observed out of 33 opportunities for error, which resulted in an error rate of 6.06%. Residents affected by these errors were 2 non-sample residents (#46, #56). The findings were: 1. Observation on 8/6/14 at 9:30 AM showed RN #1 gave resident #56 two tablets of calcium/magnesium/vitamin D 500/250/500 mg. Review of the 7/27/14 physician orders [REDACTED]. The 2 tablets that were administered was twice the amount ordered. 2. Observation on 8/7/14 at 9 AM showed RN #1 gave resident #46 Potassium liquid 15 ml (20 mEq) in apple juice. Review of the physician orders [REDACTED]. The resident was administered twice the amount of the medication as ordered. 3. Interview with the DON on 8/7/14 at 11 AM confirmed these medications were not given as ordered by the physician. 2017-08-01
