cms_WY: 1990

In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The census was 111. The findings were: Observation on 1/12/16 at 11:30 AM showed a notebook containing state survey results was posted next to the front door in a sitting area. The notebook was located approximately 5 feet off the floor with an armchair below it, putting it out of reach for residents in wheelchairs. Additionally, the most recent health survey found in the notebook was from the 7/23/15 complaint survey. The 11/6/15, 11/18/15, and 12/2/15 complaint surveys were not in the book. Interview with the administrator on 1/15/16 at 11:45 AM confirmed the survey results were not accessible to all residents, and needed to be lowered. He further confirmed the most recent survey results were not available in the notebook. 2017-09-01
