In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility failed to ensure the resident's room was in good repair, clean and /or homelike. Resident identifiers: #8, #3 and #4. Facility census: 178. Findings included: An observation on 04/17/18 at 10:30 AM, revealed a lack of personal items noted in Resident #8's room. An interview with the District Director of Clinical Services, on 04/17/18, at 12:20 PM, verified it was uncertain why Resident #8 did not have personal effects making the room homelike. It was further stated, staff would assess resident's preferences and assist the resident to make the room homelike. An observation of Resident #3's room, on 04/16/18 at 11:05 AM, revealed a plastic glove laying under a chair. An additional observation, on 04/17/18, at 7:30 AM, revealed a plastic glove laying under the same chair and debri on the floor, in the area close to the door. c) Resident #4 An observation of Resident #4, on 04/16/18 at 11:05 AM, revealed paint missing along with paint chips hanging from the ceiling above the bed. An interview with the District Director of Clinical Services (DDCS), on 04/16/18 at 11:10 AM, revealed the ceiling would be taken care of immediately. 2020-09-01
