In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

A medication re-order sheet was left unattended on medication cart in the hallway. Personal identifiers including residents' names, medications, and other health information were viewable by any person in the hall. This was a random observation. This practice affected five (5) residents. Resident identifiers: #20, #21, #22, #23, #24, and #25. Facility census: 178. Findings included: a) Medication Re-Order Sheet A random observation of the 3rd Floor B-Hall, on 04/16/18 at 11:45 AM, revealed a medication re-order sheet was left on top of the medication cart. The medication re-order sheet contained the following: -Resident #20-Resident's name, room number, medication, and dosage -Resident #21-Resident's name, room number, medication, and dosage -Resident #22-Resident's name, room number, medication, and dosage -Resident #23-Resident's name, room number, medication, and dosage -Resident #24-Resident's name, room number, medication, and dosage -Resident #25-Resident's name, room number, medication, and dosage An interview with Licensed Practical Nurse (LPN) #10, on 04/16/18 at 11:48 AM, revealed the LPN should not have left the medication re-order sheet unattended on the medication cart. The LPN stated she usually turns the paper over or takes it with her when away from the cart so that no patient information can be seen by others. 2020-09-01
