In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Foods were found stored incorrectly, and staff used the same gloves to handle food and non-food items This practice has the potential to affect more than a limited number of residents who are served from this central location. Facility census: 75. Findings included:a) During the initial tour of the dietary department at 11:00 a.m on 5/13/19 at lunch revealed the following issues. The dietary manager was present at the time of the observations. 1. Sugar was stored with the scoop being in direct contact with the product. Scoops are to be stored in a manner that the serving portion is not in contact with the product. 2. A styrofoam cup was stored directly in a plastic container in the product. The dietary manager identified it as thickened. This also should have the device used to scoop the item from the container not be in direct contact with the product itself. 3 A dietary staff member was noted to be handling fried green tomatoes with her gloved hand. The staff was also seen touching non-food items with those same gloves. This practice could lead to possible cross contamination of the foods. 2020-09-01
