In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: a) Resident #55 Review of medical records, on 02/13/12 at 10:00 a.m., found laboratory tests had been obtained for Resident #55 on 07/12/12 and 07/19/12. The physician was not notified of the results until 08/13/12. An interview, on 02/13/13 at 2:20 p.m., with Employee #113, the director of nursing (DON) and Employee #10, the administrator, confirmed that due to the changing of lab services, they had not received the results of the resident's ammonia levels, obtained on 07/12/12 and 07/19/12, until 08/13/12. 2016-02-01
