In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident identifier: #57. Facility census: 60. Findings Include: a) Resident #57 Review of the resident's current care plan, with a review date of 10/31/12, revealed the problem, Resident at risk for complications related to the use of [MEDICAL CONDITION] drugs antianxiety and antidepressant medications, with a goal of complete behavior monitoring flow sheet. Review of physician orders [REDACTED]. On 12/06/12, at approximately 3:00 p.m., the director of nursing, Employee #36, was interviewed concerning the completion of behavioral flow sheets for Resident #57. Employee #36 stated a behavioral flow sheet was not completed when a resident is on antidepressants.
