In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This had the potential to affect more than an isolated number of residents. Facility census: 64. Findings include: a) During an interview with the resident council president (Resident #13) on 07/09/14 at 9:00 a.m., the president said the facility did not deliver residents' mail on Saturdays. On 07/09/14 at 9:40 a.m., Employee #25 (business office) said the facility delivered the mail from Saturday on Monday morning. On 07/09/14 at 9:30 a.m., the activity director (Employee #59) said she worked three (3) Saturday's a month and would be glad to distribute the mail on those days. 2017-09-01
