In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This had the potential to affect more than a limited number. Facility census: 54. Findings include: a) An attempt to locate the required nurse staffing data posting during the initial tour of the facility on 02/24/14 at 12:30 p.m., revealed it was not posted. During an interview with the director of nursing (DON), Employee #12, on 02/24/14 at 12:37 p.m., she verified there was no posting of the current staffing data in the facility. On 02/26/14 at 4:30 p.m., the staffing sheet was found on a clip board at the nurses' desk with the day shift posting only. Registered Nurse (RN), Employee #4, verified the staff posting was not completed for evening shift and stated she would inform the DON.
