In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility posted the survey too high for a person sitting in a wheelchair to reach This had the potential to affect more than a limited number of residents. Facility census: 62. Findings include: a) Upon initial entry into the facility, on 09/14/14 at 4:15 p.m., the survey results were observed attached to the wall at a height too high to be accessible to a person sitting in a wheelchair. On 09/17/14 at 2:25 p.m., a resident who was sitting in a wheelchair was asked to attempt to reach the survey results. The resident was unable to obtain the survey results. At 3:45 p.m., on this same date, the Administrator was made aware the survey results were not accessible to all residents. The next day the survey results were placed on the wall within reach of all residents. 2018-05-01
