The survey results book was located on the wall at a height not accessible to residents in wheelchairs. This practice had the potential to affect more than an isolated number of residents. Facility census: 160. Findings include: a) An observation on 07/22/15 at 1:35 p.m., revealed the survey results book was located in a plastic holder that hung against a wall in the front lobby. The book was too high for residents in a wheelchair to reach. On 07/22/15 at 3:07 p.m., the Nursing Home Administrator agreed the survey book was not located at a height accessible to a resident in a wheelchair. She stated the survey book would be moved to a location accessible to residents in wheelchairs. | 2019-02-01 |
