In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #153, who was able to use the call light system, did not have his call light within reach. This was a random opportunity for discovery. Resident identifier: #153. Facility census: 108. Findings included: a) Resident #153 On 01/28/19, at 1:12 PM, the resident's responsible party and the resident said the call light was not always within reach. The resident said he frequently uses the light to call for assistance when needed. Today the call light was within reach. The responsible party said she had placed the call light where the resident could reach it when she came to visit. Observation of the resident with Registered Nurse (RN) #69 at 8:00 AM on 01/29/19, found the resident was in bed. The resident's call light was wrapped around the bed frame, dangling downward towards the floor, at the top of the bed. The resident demonstrated he could not reach or find the call light. RN #69 placed the call light beside his left arm and clipped it to the bed for easy access. 2020-09-01
