In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This affected one (1) of sixteen (16) from the sample. Facility census: 46. Finding included: a) Resident #18 On 04/02/18 a record review revealed Resident #18 returned to the facility from a local hospital with an indwelling Foley catheter on 03/30/18. The indwelling catheter was not care planned at that time. On review of records on 04/03/18 a care plan was on record showed a date created on 04/03/18. The indwelling catheter was not care planned for four (4 ) days. On 04/02/18 at 4:30 PM, the Director of Nursing (DoN) was asked for a paper copy of the care plan. On 04/03/18 at 4:45 PM, DoN the DoN provided a paper copy of the resident's care plan. At that time, the DoN was informed no care plan was developed for the indwelling catheter until four (4) days after Resident #18 returned from the hospital. 2020-09-01
