In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on observation and staff interview, the facility failed to post nursing staffing information which contained all the elements required by this regulation. In addition, the posted information was not documented in a clear, readable format. This practice had the potential to affect all facility residents, families, and guests. Facility census: 129. Findings include: a) During tour on 05/02/11 at 3:00 p.m., observation of the posted nursing staffing information found it did not differentiate between registered nurses and licensed practical nurses. Additionally, it was not documented in a clear manner that any person reviewing it could understand. The confusion associated with reading the form was confirmed by the staff scheduler (Employee #162) during an interview at 3:45 p.m. on 05/02/11. . 2014-03-01
