In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on observation and staff interview, the facility failed to assure that two (2) male dietary aides wore effective hair restraints to prevent potential contamination of food by facial hair. This unsanitary practice had the potential to affect all residents receiving an oral diet. Facility census: 80. Findings include: a) An inspection of the dietary department, during the noon meal on 12/11/10 at 12:20 p.m., noted one (1) male dietary aide (Employee #3) was assisting with the service of the noon meal. Observation found Employee #3 had approximately 1/2 inch hair growth on his lip and chin with no beard guard in use to prevent his facial hair from potentially contaminating the food being served. Further observation found another male dietary employee (Employee #4) making peanut butter and jelly sandwiches in an adjacent room. Observation found Employee #4 had facial hair on his lip, chin, and along his jaw with no beard guard in place. When asked why he was not wearing a hair restraint on his beard and mustache, he stated that he had only worked there about three (3) weeks and had never been told he needed to wear anything on his face. 2014-04-01
