Per record review and confirmed during an interview with the Director of Social Services and Director of Nursing (DNS) on 1/3/13 at 2:14 P.M., Care Plans for Residents #1, #2, #3, #4, #5, #6, #7 and #8 were not revised to reflect that the stair chair lift was not working and how this impacted the resident's activities and mobility.
