Specifically, the facility nursing staff did not notify the physician of abnormal blood glucose results. Resident Identifier: 93. Findings include: Resident 93 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. Resident 93's medical record was reviewed on 6/20/18. Resident 93's (MONTH) (YEAR) Medication Administration Record [REDACTED] a. On 06/13/18 at 4:01 PM, low blood sugar of 37 mg/dL (milligrams per deciliter) b. On 06/14/18 at 6:21 PM, low blood sugar of 45 mg/dL c. On 06/16/18 at 8:10 AM, high blood sugar of 578 mg/dL On 6/20/18 at 10:55 AM, an interview was conducted with Licensed Practical Nurse (LPN) 1. LPN 1 reported that resident 93's blood sugars are not controlled. LPN 1 stated that she reported the blood sugar on 6/19/18 of 344 to the physician. On 6/20/18 at 11:12 AM, an interview was conducted with LPN 2. LPN 2 reported that if a resident had a blood glucose level under 60 mg/dL or over 400 mg/dL, she would notify the physician. LPN 2 stated if there was an order to notify the physician with a different parameter for blood glucose, she would follow the order. On 6/20/18 at 3:20 PM, an interview was conducted with the Director of Nursing (DON). The DON stated that a resident's physician should be notified if a resident's blood glucose level was less than 60 or over 400. The DON did not provide additional information that the physician was contacted regarding the abnormal blood glucose levels on 6/13/18, 6/14/18 and 6/16/18. | 2020-09-01 |
