In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident 191's medical record was reviewed on 8/16/16. A physician's orders [REDACTED]. A review of resident 191's (MONTH) (YEAR) MAR indicated [REDACTED] a. On 8/2/16 at 5:30 PM, no documentation was present for the administration of the [MEDICATION NAME]. b. On 8/2/16 at 3:30 PM, no documentation was present for the administration of the [MEDICATION NAME]. c. On 8/2/16 at 5:00 PM, no documentation was present for the administration of the carvedilol. d. On 8/2/16 at 5:00 PM, no documentation was present for the administration of the [MEDICATION NAME] sulfate. (Note: Resident 191 was admitted to the facility on [DATE] at 1:39 PM.) A physician's orders [REDACTED]. A review of resident 191's (MONTH) (YEAR) MAR indicated [REDACTED] a. On 8/12/16 at 11:30 AM and 4:30 PM, no documentation was present for the administration of the insulin. b. On 8/13/16 at 11:30 AM and 4:30 PM, no documentation was present for the administration of the insulin. c. On 8/16/16 at 11:30 AM and 4:30 PM, no documentation was present for the administration of the insulin. On 8/17/16 at 9:50 AM, an interview was conducted with the Director of Nursing (DON). The DON stated that she had interviewed the nursing staff and the nursing staff confirmed that they had administered the medications as prescribed to resident 191. On 8/17/16, late entry documentation notes were provided by the DON to justify that the medications in question were administered by the nursing staff as prescribed by the physician. 2020-09-01
