In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Observation on 7/23/18 at 10:20 AM, with the Dietary Manager (DM) present, in the dietary department revealed 17 of 17 tray delivery carts had a build-up of calcium on the interior and the tray rungs. Further observation revealed 16 of the 17 tray delivery cart interiors had rust present. Further observation revealed the interior of the dish machine had a heavy accumulation of calcium. Further observation of all the insulated plate dome lids and insulated heated plate bases interior and exterior had heavy accumulation of calcium. The calcium deposits on the insulated heated base could interfere with the base heating process and therefore could fail to maintain the food temperatures. The calcium deposits on the insulated dome lid and base could prevent a good seal to maintain the food temperature. Interview with the Dietary Manager on 7/23/18 at 10:20 AM in the dietary department confirmed the facility failed to maintain the tray delivery carts to prevent calcium build-up and to prevent rusting. Further interview confirmed the facility failed to maintain the interior of the dish machine from building up calcium. Further interview confirmed the insulated dome lids and bases had an accumulation of calcium present. Interview with the Maintenance Director on 7/23/18 at 10:50 AM in the dietary department confirmed the dietary department water left calcium deposits inside the dish machine. Further interview revealed the dish machine .is old and breaks down frequently .and needs the conveyor belt replaced . Further interview confirmed calcium deposits were present on the resident insulated plate dome lid and base making them .look unattractive . 2020-09-01
