In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on observation and interview, the facility failed to serve pureed food at the appropriate consistency to 22 of 22 residents receiving pureed textured food. Findings include: Observation on 7/23/18 at 11:40 AM in the dietary department, with the Dietary Manager present, revealed the resident mid-day meal tray service was in progress. Further observation revealed the pureed textured beef, potatoes, and cauliflower all pooled together in the plate. Interview with the Dietary Manager on 7/23/18 at 11:40 AM in the dietary department confirmed the facility failed to serve pureed textured food at an appropriate consistency and appetizing manner. |
