In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings included: Observation on November 20, 2013, at 7:30 a.m., in the dietary department revealed an approximately four inch wide stream of water from the pressure steamer running on the floor to the trayline in operation. Further observation revealed dietary staff walking in and through the area of the water. Interview on November 20, 2013, at 7:30 a.m., with the dietary chef present during the observation, revealed the pressure steamer had .been leaking a long time . Interview on November 20, 2013, at 7:45 a.m., with the Certified Dietary Manager, by the pressure steamer, confirmed the steamer was leaking onto the floor and the water was crossing the floor to the trayline area. 2017-05-01
