In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility has 92 of the 96 residents who receive a meal tray from the kitchen. The findings included: 1. Review of the facility's Safety & (and) Sanitation Best Practice Guidelines policy documented, .Sweep and spot mop after each meal and as needed; thorough mopping daily; scrubbing/brushing weekly . Review of the facility's kitchen cleaning schedule documented, .sweep, mop, clean and straighten . Observations in the kitchen on 2/2/15 at 10:28 AM and 2/3/15 at 7:50 AM, revealed the presence of dark brown, gritty dust buildup under the edge of freezer #1. 2. Observations in the kitchen on 2/3/15 at 10:14 AM, revealed RHIT and CNA #1 in kitchen area without a secured hair. Observations in the hall outside the kitchen on 2/4/15 at 9:31 AM, revealed signs PATIENTS RING BELL FOR SERVICE and DIETARY SERVICES. Observation revealed hair restraint covers available in the hall before entering the door to the kitchen area. 2018-07-01
