In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings included: Resident #2 was readmitted to the facility on [DATE], with [DIAGNOSES REDACTED]. Medical record review of the Minimum Data Set (MDS) dated [DATE] revealed the resident had decreased range of motion and partial loss of movement on one hand. Medical record review of the MDS dated [DATE] revealed the resident was severely impaired with decision-making skills and totally dependent on staff for all activities of daily living. Continued review revealed the resident had decreased range of motion and partial loss of movement on one hand. Medical record review of the care plan dated September 16, 2010 revealed, "PT/OT to eval & tx (physical therapy/occupational therapy to evaluate and treat)." Medical record review of a physician's orders [REDACTED]. Medical record review revealed a physician's orders [REDACTED]. Medical record review of a physician's orders [REDACTED].(joint) contractures ...pulls away from this therapist on attempts at PROM (passive range of motion) but ultimately allowed gentle PROM ...and application of palmar rolls ..." Observation on September 23, 2010 at 10:55 a.m. revealed the resident in bed and without any contracture preventive device in either hand. Observation on September 24, 2010 at 10:35 a.m. revealed the resident in a geri-chair in the room and hand rolls in the right and left hands. Interview with OT #1 on September 24, 2010 at 10:58 a.m. in a lower level conference room revealed a communication error had occurred regarding the physician's orders [REDACTED].#2. C/O: # 2014-01-01
