In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings included: Resident #6 was admitted to the facility on [DATE], with [DIAGNOSES REDACTED]. Medical record review of a physician's orders [REDACTED]. Continued medical record review of the resident's care plan dated January 13, 2011, revealed " ...wounds currently healed..." Observation on January 25, 2011, at 10:45 a.m., and January 26, 2011, at 3:52 p.m., revealed the resident lying in bed with an indwelling catheter in place. Interview with the Director of Nursing on January 26, 2011, at 4:05 p.m., at the AB hall nursing desk confirmed the resident's wound was healed and the catheter was not removed. 2014-12-01
