In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings included: Medical record review for Resident #15 documented an admission date of [DATE] with [DIAGNOSES REDACTED]. Review of the physician's orders had no order documented for the following laboratory tests obtained: a. Basic Metabolic Panel (BMP), Lipid Profile (LP) and [MEDICATION NAME] level on 4/12/11. b. Hemaglobin A1C on 5/16/11. c. [MEDICATION NAME]/Internationalized Ratio (PT/INR) on 7/22/11, 8/10/11, 8/22/11, 8/25/11 and 8/30/11. During an interview in the Assistant Director of Nursing's (ADON) office on 9/14/11 at 9:50 AM, the ADON was asked about the PT/INR's with no orders. The ADON stated, "...did too many, we didn't pull it (lab slips) out of the file... should not have been done..." The DON was asked about the BMP, LP and [MEDICATION NAME] level on 4/12/11 and the Hemaglobin A1C on 5/16/11. The ADON stated, "...I don't have orders for those..." 2015-02-01
