In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings included: Review of the facility's "Treatment/Wound Cleansing/Dressing changes" policy documented, "...14. Cleanse wound well ...16. ...remove your dressing field and throw in double bag garbage along with your gloves. 17. Wash hands..." Observations in Random Resident (RR) #1's room on 9/14/10 at 9:05 AM, revealed Nurse #1 cleansed the wound on RR #1's left heel with 4 by (x) 4's and wound cleanser. Nurse #1 applied [MEDICATION NAME] and the new dressing. Nurse #1 did not wash her hands between cleaning the wound and application of the medication and the new dressing. During an interview on the south hall 9/14/10 at 9:20 AM, Nurse #1 was asked why she did not wash her hands after cleaning the wound. Nurse #1 stated, "I never have done that (wash hands after cleansing the wound) before..." 2015-05-01
