In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: 1. Review of resident 54's Discharge Summary revealed: *She had been discharged to home on 6/24/18. *A brief nursing summary had been written. *The document asked for a brief therapy, social services, activity, and dietary narrative summary. *All those areas had been left blank. Interview on 9/13/18 at 10:19 a.m. with the director of nurses regarding the above for resident 54 revealed: *When a resident was discharged she sent a request to the appropriate disciplines to complete the Discharge Summary. *It was her expectation for them to complete that document. -That had not been done for the resident. *A request for the policy on discharge summaries was made at that time. -None was received by the end of the survey. 2020-09-01
