Review of the provider's QA meeting minutes revealed the last QA meeting had been held on 10/31/12. The QA review had been for 7/1/12 through 9/30/12. There had been no QA meetings since then. 3. Interview on 2/14/13 at 9:00 a.m. with the administrator revealed: *She was not aware the medical director had not attended all of the QA meetings. *She had not attended the 8/1/12 QA meeting. *She stated the next QA meeting was scheduled for 2/27/13. *The next QA meeting had been scheduled for that time due to conflicts in scheduling.
