The findings included: The facility admitted Resident #2 on 9/21/09 with [DIAGNOSES REDACTED]. During the initial tour on 7/6/10 at approximately 11:30 AM, the resident was observed in bed with his feet extended over the foot of the bed. During all days of the survey, the resident was observed in bed in the same situation , either with a folded towel or pillow under his feet. In an interview with a family member on 7/6/10 at 8:25 PM, he/she stated that when visiting, he/she would try to pull the resident up in order to be more comfortable. In an interview with the Administrator, Director of Nursing and Nursing Consultant on 7/7/10 at 10:35 AM, they stated that the resident would slip down in the bed. A later observation revealed a blue foam wedge between the end of the mattress and the foot of the bed. with the resident's feet on the wedge. | 2014-09-01 |
