In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

One of one investigation of alleged misappropriation of funds was not reported to the State Survey and Certification Agency. The findings included: On 02/27/2012 an unannounced visit was made to the facility to investigated a facility reported allegation of misappropriation of one resident's funds. During the investigation the facility provided a summary of their investigation. Review of the investigation revealed the facility had not obtained a statement from the alleged perpetrator. The summary stated they had discussed the allegation with the perpetrator and she wanted the facility to make a payment plan. During an interview with the Executive Director, and Administrator on 3/2/12 at 9:20 AM, they confirmed they had not obtained a statement from the alleged perpetrator.
