In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

(Resident #15, #2 and #4). The findings included: Cross refer to F281, Standards of Practice related to documentation Resident #15 was sent to the emergency room . There was no documentation of the residents condition, assessment of a change of condition or the medical need for a transfer. The resident did not have complete wound/skin documentation. Review of the medical record revealed that the documentation of the sacral area wound was not adequate to determine if the wound had worsened or healed. Resident #2 had both Patient with Ability to Consent and Patient Without Ability to Consent forms on her/his medical record. Both were signed on the same days by the resident representative and the Physicians. A physician's orders [REDACTED]. Resident #4 had a physician's orders [REDACTED].
