In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility failed to conduct a thorough investigation, to obtain written statements and failed to report allegations of abuse/neglect to state agencies. The findings included: Cross-refer to F-225 Investigating and Reporting allegations of abuse neglect. Resident #4's family reported allegations of misappropriation (stolen money) and neglect (not being changed; Resident #5 family reported concerns related to incontinent care; Resident #7's family alleged neglect related to the failure of the facility to provide incontinent care for approximately 4 1/2 hours. Review of the facility Abuse Policy and Procedure (P&P) revealed the Policy stated, This facility does not tolerate any forms of mistreatment, neglect, abuse or misappropriation of resident property. Each resident has the right to be free from verbal, sexual, physical, and/or mental abuse, corporal punishment, and involuntary seclusion. This policy is adhered to whenever an incident or suspected incident of verbal or physical abuse, neglect, or mistreatment/misappropriation of resident property (or other reasonable suspicion of a crime) has occurred. The P&P defined Neglect. Neglect means failure to provide goods and services necessary to avoid physical harm, mental anguish or mental illness. Under the title Procedure the section on reporting stated, C. The Administrator or designee immediately begins investigating the allegation(s). The Department of Health and Environmental Control and the ombudsman are also notified, as appropriate. The initial report must be phoned or faxed in within 24 hours. Under the Section entitled Investigations: Section B. Interviews will be conducted with all pertinent parties and written and signed statements obtained from those persons, who saw or heard information pertinent to the incident. If residents are able to tell you what happened, write out their statements, review them with the residents and have them sign. Review of the grievance investigations revealed the facility did not obtain written statements from persons involved in the allegation that may have had relevant information. 2016-05-01
