In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

On days of the survey, based on review of residents' funds and interview, the facility failed to complete 3 of 3 mandated Liability Notices: Skilled Nursing Facility Advanced Beneficiary Notices (SNFABN) for 3 of 3 residents. The findings included: On 1/23/13 at approximately 10:30 AM, review of 3 of 3 residents' funds revealed mandated Liability Notices had not been completed. During an interview on 1/23/13 at approximately 10:30 AM with the Director of Social Services/Activities, s/he confirmed the Skilled Nursing Facility Advanced Beneficiary Notices (SNFABN) were not completed. |
