In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #3 did not receive Restorative Nursing as recommended by Occupational Therapy. The findings included: The facility admitted Resident #3 with [DIAGNOSES REDACTED]. Review of the Quarterly Minimum Data Set((MDS) dated [DATE] revealed the resident had range of motion limitations of the upper and lower extremities on one side. Review of the Occupational Discharge Note dated 6/22/12 revealed a recommendation for Restorative Nursing for left upper extremity range of motion and to continue with a rolled washcloth in the left hand at all times as tolerated except for patient care. Further review of the medical record revealed no restorative notes on the record. On 7/24/12 the Unit Manager was asked if she could locate documentation related to restorative services. The Unit Manager stated that Restorative Nursing had not been initiated. Further interview with Administrative Staff revealed that Restorative had not been started due to failure of therapy not initiating the paperwork. 2016-06-01
