**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** On the days of the survey, based on record reviews, the facility failed to ensure that comprehensive care plans were developed for residents with tracheostomies related to type and size of cannulas needed, availability of back-up [MEDICAL CONDITION], and emergency procedures in the event of decannulation for 2 of 2 residents reviewed who had tracheostomies (#2 and #13). The findings included: Review of the plans of care for resident #2 and resident #13 revealed that their [MEDICAL CONDITION] status was included in various problems, however, the care plans did not address what types of cannulas the residents required, the size needed, the location of emergency replacement cannulas, or emergency procedures to be used in the event of decannulation.
