In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

7965 CHERAW HEALTHCARE 425005 400 MOFFAT ROAD CHERAW SC 29520 2012-07-18 253 E 0 1 RSSS11 On the days of the survey, based on observations, interviews and review of the facility's policy for Infection Control (Housekeeping/Laundry), the facility failed to maintain sanitary conditions for 7 out of 10 shower rooms examined as evidenced by black and pink substances noted on multiple shower chairs, hair and dark spots on multiple shower floors and unpleasant odors detected in 2 of 7 shower rooms. The findings included: Observations on 7/17/12 at approximately 11:30 am of the shower room on the right side of the hall containing rooms 100-107 revealed a shower stall with a yellowish substance and dark spots on the shower floor. Observations on 7/17/12 at approximately 11:32 am of the shower room on the left side of the hall containing rooms 100-107 revealed a shower stall with dark spots on the floor of the shower. Observations on 7/17/12 at approximately 11:35 am of the shower room on the hall containing rooms 108-122 revealed a navy blue shower chair with a black substance in a large number of the piping joints, under the mesh seat on the piping, and above the wheels. There was also noted a strong unpleasant odor within the room and dark spots on the privacy curtain. Observations on 7/17/12 at approximately 11:40 am of the shower room on the hall containing rooms 123-134 revealed the shower stall with dark spots and hair on the floor of the shower. Within the shower stall was a navy blue shower chair with a whitish/pink substance on the back of the shower chair. Also, the navy blue shower chair had a black substance in multiple joints of the piping. There was also noted an unpleasant odor within the room and the privacy curtain had dark spots on the fabric. Observations on 7/17/12 at approximately 11:50 am of the shower room on the left side of the hall containing rooms 200-207 revealed the shower stall floor had dark spots and within the shower stall was a green shower chair with a black substance along the mesh seam of the back of the chair. Observations on 7/17/12 at approximately 11:55 am of the shower room on the right side of the hall containing rooms 223-234 revealed the shower stall had dark spots and hair on the floor. On 7/18/12 at approximately 9 am a follow up tour was conducted for each shower room listed above with no changes in the above stated condition being noted for each of the shower rooms. During an interview on 7/18/12 at approximately 9:10 am the Housekeeping/Laundry Supervisor stated that housekeeping cleaned the shower rooms once lunch trays were passed out on the halls, around 12 noon each day. She was shown each of the shower rooms in question and stated the shower rooms were not as clean as expected. The supervisor described the dark spots on the floor of the shower as a dirty film, and stated she saw the hairs on several of the shower stall floors and did detect an odor in 2 of the shower rooms. During an interview on 7/18/12 at approximately 9:10 am the Maintenance Supervisor stated that the maintenance department was responsible for cleaning the shower chairs and replacing the privacy curtains for each unit, but only when asked to do so by the staff. He stated that they do not have a set schedule to clean the shower chairs or replace the privacy curtain for the shower rooms. An interview with the Director of Nursing (DON) on 7/18/12 at approximately 9:25 am revealed that she expected the facility to maintain clean shower rooms. After showing the DON the shower room on hall 108-122, she verified the black substance in the piping joints of the shower chair and stated the chair did need cleaning and she would not want to use it. She also stated she did detect a urine odor in the room and did see the dark spots on the privacy curtain. Upon review of the facility's policy for Infection Control (Housekeeping/Laundry). The facility stated that the workplace would be maintained in a clean and sanitary condition with a written schedule of cleaning and decontamination. It also stated that it was important that a clean, safe and sanitary environment is maintained for their residents. The policy stated that the horizontal surfaces such as sinks, tubs shower floors toilet seats, floors, etc will be cleaned daily using an approved Environmental Protection Agent (EPA) disinfectant detergent solution. Also, these surfaces will also be cleaned as needed when spills or soiling occur. The policy stated that other surfaces such as walls, curtains, blinds, etc. will be done when dust/soil is visible. 2016-09-01
