In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. Resident #3 had no air mattress as ordered. The facility failed to administer treatments as ordered for Resident #8. Further record review revealed a physician's orders [REDACTED]. Observation of Resident #3 on 10/12/13 revealed no air mattress in place as ordered. CNA(Certified Nursing Assistant) #2 confirmed the resident was not lying on an air mattress at that time. On 10/12/13 at 3:15 PM, RN (Registered Nurse) #2 confirmed the resident was not lying on an air mattress and no explanation was given as to why the resident was not on an air mattress at the time of the survey. The facility admitted Resident #8 with [DIAGNOSES REDACTED]. Review of the TAR's(Treatment Administration Record) for July 2013 and August 2013 revealed the following treatment omissions: 1)Clean open area to sacrum with NS(Normal Saline). Pat dry. Apply [MEDICATION NAME] change q(every) 3 days and prn (as needed) until healed: 7/29/13; 2) Cleanse area to sacrum with NS, pat dry apply Hydrogel to wound bed cover with [MEDICATION NAME] and change qd (every day) until healed-8/3-6/13. 2016-10-01
