In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #1 had a blood sugar over 500 and the physician was not notified. The findings are as follows: The facility admitted Resident #1 with [DIAGNOSES REDACTED]. Review of the resident's Physician Cumulative Orders revealed an order for [REDACTED].>(greater than) 500 give 12 units of Insulin SQ (subcutaneous) and call (the doctor). Review of the Diabetes Monitoring Record revealed on 8/7/13 at 7:00 AM, the resident's blood sugar was 529. The resident was given the 12 units of insulin but the doctor was not notified. On 10/2/13 at approximately 1:45 PM the Director of Nursing (DON) was interviewed by the surveyor. The resident's blood sugars were reviewed with the Director of Nursing. After reviewing the medical record, the DON confirmed the physician had not been notified of the 529 blood sugar. 2016-10-01
