In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This failure placed residents at risk to not have their care needs met due to a lack of qualified nursing staff. Findings include: In an interview on 04/12/17 at 4:25 p.m., Assistant Director of Nursing (Registered Nurse) #3 explained she worked Monday through Friday, supervising staff and ensuring care needs were met. She also explained the facility employed three RNs who worked the floor. At 4:30 p.m., review of staffing schedules for 03/25/17 through 04/02/17 revealed on Saturday (03/25/17 and 04/08/17) and Sunday (03/26/17 and 04/09/17) the facility had no RN coverage. At 4:35 p.m., Assistant Director of Nursing 3 stated If that is what it says, then that is what it is. In an interview on 04/13/17 at 7:57 a.m., Scheduler #107 explained the facility previously had three RNs, however two recently quit and so there was only one currently on the schedule. She explained she was not aware of the requirement to have an RN scheduled every day. I just schedule what I have. 2020-09-01
