In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings include: On 10-15-2012 at approximately 11:50 AM, inspection of the 2 North medication room revealed 5 oxygen tanks sitting on the floor which were unsecured by any means to the wall or floor. RN (Registered Nurse) # 1 verified this finding on 10-15-2012 at approximately 11:57 AM and stated that the oxygen tanks had just been resupplied and should have been secured in a safe manner in the metal storage rack. During initial tour on 10/15/12 of Unit 1 South, the oxygen storage closet was unlocked and one unsecured oxygen tank was noted near the door. 2017-01-01
