In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The observations had the potential to affect all resident's serviced by the affected areas. The findings include: During an initial tour of the kitchen on 1/21/14 at approximately 12: 23 PM, the surveyor observed 2 loaves of bread stored with the used by date of 1/18/14. Further observation of the kitchen on 1/21/14 revealed dust and grease build up on the back splash of the stove and staff with facial hair walking through the kitchen without wearing a beard restraint. On 1/23/14 at approximately 10:27 AM, during a repeated tour, the surveyor observed the can opener soiled with metal shavings and debris on the blade. During an observation of the dining rooms in the facility on 1/23/14 at approximately 11:00 AM, the surveyor observed 2 of 2 microwaves on each unit with a build up of food splatters and debris. During an interview with the Food Service Director on 1/23/14 at approximately 12:03 PM, s/he confirmed the surveyor's findings. 2017-09-01
