In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #7's care plan did not reflect the alleged incident of sexual abuse and interventions to ensure safety of the resident from another resident's visitor. Cross Refer to F 226 as it relates alleged abuse of Resident #7 and failure of the facility to put interventions in place to protect the resident. The findings included: Review of Resident #7's Care Plan on 09/04/14 at approximately 3:30 PM revealed that the Care Plan was reviewed and revised by the Interdisciplinary Team, including Social Services and Nursing. An allegation of abuse related to inappropriate touching by a visitor was reported on 06/12/14. Resident #7's Care Plan was documented as reviewed before the incident on 04/17/14 and after the incident on 07/15/14. During an interview on 09/04/14 at approximately 3:30 PM with the Minimum Data Set (MDS) Assessment Nurse confirmed that the Care Plan received by the surveyor for Resident #7 was current and was not updated to include specific interventions to protect the resident and prevent recurrence. 2018-04-01
