In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings included: The facility admitted the resident with [DIAGNOSES REDACTED]. Record review on 7/28/15 revealed the resident had a physician order [REDACTED]. The resident was transported to the hospital the day after admission. On return to the facility, the resident's cognitive level had changed. No new documents had been filled out by the facility. During an interview on 7/28/15 with the Social Service Director, he/she stated if residents were not able to make their own decisions, the facility did not have 2 physicians on staff nor any way to be able to have 2 physicians to determine if residents were able to make their own health care decisions. At the time of readmission, the resident had confusion and a BIMS (Brief Interview for Mental Status) score of 7. Interview with the Director of Nurses and LPN (Licensed Practical Nurse) #1 revealed both nurses would look at the physician's orders [REDACTED].
