The findings included: The facility admitted Resident #35 on 7-15-15 with [DIAGNOSES REDACTED]. Record review on 10/10/18 at 11:53 AM revealed physician's orders [REDACTED]. Review of laboratory reports and Progress Notes on 10/10/18 at 12:43 PM revealed no evidence of active infection. Review of Physician's Progress Notes revealed no documentation of continued need or evaluation of risks versus benefits of long-term [MEDICATION NAME] antibiotic use. During an interview on 10/11/18 at 1:47 PM, the Pharmacist verified the physician's orders [REDACTED]. During an interview on 10/10/18 at 2:49 PM, when asked about the continued use of the antibiotic, the Assistant Director of Nurses stated that the resident had been admitted on an antibiotic for UTIs.
