In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

(Resident #13) The findings included: The facility admitted Resident #13 on 2/7/17 with [DIAGNOSES REDACTED]. Record review revealed the facility submitted an Initial 2/24-Hour Report of an allegation of Misappropriation of Resident Property concerning Resident #13 by fax to the State Agency on 5/23/18. When asked for a copy of the Five-Day Follow-Up Report and documentation of the date submitted to the State Agency, the facility informed the surveyor that the report was not submitted. During an interview on 8/10/18 at approximately 9:45 AM, the Administrator confirmed this finding. Review of the facility's policy entitled, Abuse & Neglect Prohibition indicated under Reporting and Response 1. STATE REPORTING OBLIGATIONS: 5. The facility will submit a summary of its investigation to the appropriate State agency within 5 days of its initial report or within whatever time frame required by the State Agency. 2020-09-01
