Findings are as follows: The facility's policy, titled, Storage of Medications states, in part, Drugs and biological's shall be stored in the packaging, containers or other dispensing systems in which they are received. Drug containers that have missing, incomplete, improper, or incorrect labels shall be returned to the pharmacy for proper labeling before storing. Additionally, the facility's policy, titled, Administering Medications states, in part, The expiration/beyond use date on the medication label must be checked prior to administering. When opening a multidose container, the date shall be recorded on the container. 1. Surveyor observation of the 1st floor nurse's medication cart, in the presence of Staff Nurse A on 12/13/2018 at 9:26 AM, revealed 6 of 12 opened and undated insulin pens, which include the following: -(1) [MEDICATION NAME] FlexTouch Pen (manufacturer's instructions reveal it can be stored after opened for 42 days). -(1) Toujeo [MEDICATION NAME] Pen (manufacturer's instructions reveal that it can be stored after opened for 56 days). -(2) Humalog KwikPen (manufacturer's instructions reveal that it can be stored after opened for 28 days). -(2) [MEDICATION NAME] Pen (manufacturer's instructions reveal that it can be stored after opened for 28 days). The facility failed to record the date the pens were opened, therefore the discard date could not be determined. Additionally, the following was observed in the same medication cart: -(1) [MEDICATION NAME] Pen with a pharmacy label for Resident ID# 57 inside a plastic bag which was labeled with another resident's name. -(1) [MEDICATION NAME] Pen with a pharmacy label for Resident ID# 65 inside a plastic bag which was labeled with another resident's name. -(1) Humalog KwikPen with a torn pharmacy label missing the resident's name inside a plastic bag. During a surveyor interview immediately after the observation with Staff A, she was unable to explain why the insulin pens were open and not dated. Further, she acknowledged that the above insulins were stored in bags that did not match the pharmacy labels and that one pharmacy label did not have the resident's name visible. 2. Surveyor observation of the 3rd floor medication room, in the presence of the 3rd floor Unit Manager on 12/13/2018 at 10:18 AM, revealed 1 of 2, multidose vials of [MEDICATION NAME] Purified Protein ([MEDICATION NAME]) was opened and undated. Manufacturer's instructions written on the box state, discard opened product after 30 days. During a surveyor interview immediately following the observation with the Unit Manager, she acknowledged that the vial was not dated when opened. You are hereby formally notified that where the above listed deficiencies also constitute non-compliance with applicable provisions of the Rules and Regulations for Licensing of Nursing Facilities they are deficiencies under State Regulations and grounds for licensure sanctions. | 2020-09-01 |
