In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: 1. Record review of a hospital continuity of care form dated 11/10/2018 for Resident ID# 26 revealed, s/he was sent to the hospital for a fall. 2. Record review of a nursing progress note for Resident ID# 35 revealed, s/he was sent to the hospital for right foot [MEDICAL CONDITION] on 8/17/2018. 3. Record review of a nursing progress note dated 10/3/2018 for Resident ID# 48 revealed, s/he was sent to the hospital for an infection. 4. Record review of a nursing progress note dated 11/7/2018 for Resident ID# 76 revealed, s/he was sent to the the hospital for a fall. 5. Record review of a social worker progress note dated 11/15/2018 for Resident ID# 219 revealed, s/he was sent to the hospital for unstable gait, slurring words and lethargy. During an interview with the Director of Nursing Services on 12/13/2018 at 2:30 PM, she was unable to provide evidence that the facility sent a notice of discharge to the ombudsman following these hospitalization s. 2020-09-01
