Findings are as follows: The surveyors obtained water temperatures on all units on 8/5/2019. The following temperatures were observed on Mystic Unit (non-dementia care unit), using digital thermometers: -10:40 AM, room [ROOM NUMBER], bathroom sink measured at 125.2 F -10:44 AM, Mystic Unit, common bathing room sink measured 125.1 F -10:48 AM, room [ROOM NUMBER], bathroom sink measured 128.2 F -10:52 AM, room [ROOM NUMBER], bathroom sink measured 125.2 F -11:35 AM, room [ROOM NUMBER], bathroom sink measured 126.0 F -11:35 AM, room [ROOM NUMBER], bathroom sink measured 126.7 F -11:35 AM, room [ROOM NUMBER], bathroom sink measured 125.2 F -11:35 AM, room [ROOM NUMBER], bathroom sink measured 121.3 F -11:37 AM, room [ROOM NUMBER], bathroom sink measured 120.4 F -11:39 AM, room [ROOM NUMBER], bathroom sink measured 122.2 F During a surveyor interview on 8/5/2019 at 11:55 AM, with the Maintenance Director, he revealed that he was unaware of the elevated water temperatures. Additionally, he revealed that he completes weekly water temperature monitoring and logs of 2 different rooms per unit and at the 2 mixing valves weekly, on Thursdays. He revealed that he has not had problems with high temperatures and expects the water temperatures to measure between 100.0 F to 110.0 F, plus or minus two degrees. Further, he expects the staff to inform maintenance if the water feels too hot. Additional measurements of the water temperatures by both the surveyor and the Maintenance Director on 8/5/2019, using separate thermometers, revealed the following: -12:05 PM, Mystic Unit, common bathing room sink measured 122.1 F -12:07 PM, room [ROOM NUMBER], bathroom sink measured 122.0 F Review of water temperature logs from 5/9/2019 to 8/1/2019 revealed room water temperatures between 104.0 F and 108. F. Surveyor interviews conducted on 8/5/2019 with the residents residing in rooms with high water temperatures, revealed the following: -10:40 AM, Resident ID #58 revealed that the water temperature is perfect in the room and took a shower in the Mystic Unit bathing room this morning and it was not too hot -10:52 AM, Resident ID #12 offered no complaints regarding water temperatures -11:35 AM, Resident ID #61 revealed that s/he adjusts the temperature to preference and offered no complaints During a surveyor interview with Nursing Assistant (NA), Staff A, on 8/5/2019 at 11:51 AM, she revealed that she has not had issues with the water temperature, the residents have not complained, and she always checks the temperature and adjust it accordingly. Additionally, if the water was too warm, she would remove the resident and report the issue to the nurse who would contact the Maintenance Department. During an additional interview with NA, Staff B, on 8/5/2019 at 11:55 AM, she revealed that she has not had any resident complain that the water is too hot. During a surveyor interview with the Administrator on 8/5/2019 at 12:12 PM, he revealed that he was unaware of the elevated water temperatures. Additionally, he expects the temperatures to be logged weekly and if there was a problem the staff would notify management. During a surveyor interview on 8/5/2019 at 11:55 AM, with the Maintenance Director, he revealed that he was unaware of the elevated water temperatures.
