In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: 1. The lower third of doors with rough edges and/or missing paint included: First Floor A) Resident rooms 109, 111, 115, 116 and 117. B) Both separate entrance doors to the main dining room. C) The entrance door to the main shower room adjacent to room [ROOM NUMBER]. Second Floor A) Resident rooms 204, 205, 219, 221, 222, 223, 224. B) The soiled utility closet adjacent to room [ROOM NUMBER]. Third Floor A) Resident rooms 302, 305, 306, 308, 310, 311, 313, 314, 316 and 317. B) The common shower room adjacent to room [ROOM NUMBER]. 2. Handrails with worn/stripped wood finishing included: First Floor: A) The left hand rail adjacent to the elevator (5' x 7 area). B.) The left and right corner edges opposite nursing station (2) (1' x 6 areas). Second Floor: A) The left and right hand rails adjacent to the elevator (2) (2' x 8 areas). B) The handrail adjacent to room [ROOM NUMBER] (10' x 8 area). C) The handrail adjacent to room [ROOM NUMBER] (1.5' x 8 area). During an interview on 6/18/2018 at 11:30 AM, both the Administrator and Maintenance Director acknowledged the above listed doors and handrails were in need of repair. 2020-09-01
